Feedback from patients is actively encouraged.  Please feel free to convey your thoughts, impressions and comments directly to your doctor, other health provider, or to reception staff.  Comments can be in writing or verbally.  There is a Suggestion Box available for your comments at the Reception



If you have a problem, we would like to hear about it.  Please feel free to talk to your doctor or receptionist.  Alternatively, you may ask for a complaints form and forward to the Manager.

Emails Policy

Emails used by the practice to transmit information outside the practice are only used at the doctors discretion.

Communication is sent via secure messaging  which is  not possible when emailing patients.

The practice takes reasonable steps to protect the privacy and security of information when it is transmitted or disclosed outside the organisation.

NFP follows RACGP recommendations to reduce the risk of interception of data and sending emails to incorrect addresses, including:

-verification of the patient’s email address

-the patient must first email the practice email requesting the information and only then is a reply sent to that email.  Patient is aware that the surgery will be responding to their email.

-obtaining patient consent and re-inforcement via patient emailing the surgery first.  Eliminates incorrect email addresses.

-results are not emailed to the patient.  Patient must either ring and speak to the nurse and if a copy is required the patient must pick up from reception.

-use of secure messaging facilities between practices where available.

Your Personal Health Information

Your doctor’s information about your past and present health in order to provide you with high quality care.

Information is called ‘personal health information’ if it concerns your health, medical history, past or future medical care and if someone reading it would be able to identify you.

This practice follows the guidelines of the ‘Handbook for the Management of Health Information in Private Medical Practice’.  This means that your personal health information is kept private and secure.

Your doctor will do his/her best to make sure that your medical records are:

Accurate, Comprehensive, well-organised, legible and secure.

Providing your information to other doctors

The doctor(s) in this practice respect your right to decide how your personal health information is used.

It is important that other people involved in your care, such as other doctors or health professionals, are informed of relevant parts of your medical history so they can best care for you.

Providing your information to others

Your doctor will not disclose your personal health information to a third party unless:

you have consented.

Using health information for quality improvement and research.

We use patient health information to assist in improving the quality of care we give to all our patients by reviewing the treatments used in the practice.

Security of information in the practice

We will ensure that any of your personal information that is put on computer is kept private the same way as occurs with paper records.

Your access to your health information

You have access to the information contained in your medical record.  You may ask your doctor about any aspect of your health care including information in your record.

Information in your record can be provided to you by way of an accurate and up to date summary of your care, for instance if you are moving away and are transferring to a new doctor.  You will be asked to apply in writing.
